T ITLE OF INFORMATION COLLECTION: OMB No. 0917-0036, Indian Health Service (IHS) Website ICD-10 Stakeholder Readiness Survey – ICD-10 Project Needs Assessment

PURPOSE: The IHS ICD-10 Project conducts extensive outreach on the ICD-10 transition to the sites in the Indian health system and would like to survey individuals at the sites for effectiveness. The collection of information about ICD-10 readiness will enable the project team to emphasize outreach in areas identified in the survey. The team will send out two surveys, in February 2014 and August 2014, and use the comparison of the results of the two surveys to gauge the effectiveness of outreach methods and the overall readiness of IHS for the ICD-10 transition. Addendum: With the one year delay of ICD-10 to a compliance date of October 1, 2015, the ICD-10 Project Team would like to send out two additional surveys; one in February 2015 and the final in August 2015.

DESCRIPTION OF RESPONDENTS: Individuals (stakeholders) who are involved with or impacted by the ICD-10 transition.
