Introduction:
Hello, my name is _______ from SSRS [INSERT INTRODUCTION HERE], and we are inviting you to take
part in a research study which will involve answering some questions over the phone. The questions are
designed to help understand children’s interactions on playgrounds in order to improve playground
safety.
This study is administered on behalf of the Consumer Product Safety Commission.
There are no risks in taking part in this survey, and your participation is voluntary. This means that you
are free to choose not to take part, or to skip certain questions. There is no penalty if you choose not to
respond. However, your complete participation will help us to understand children’s exposure to
playground elements, and to improve their safety. It is important that as many people respond to this
survey as possible so that the information we get is complete.
Your personal information will be kept separate from your survey responses. We do not need to know
your name and any contact information will be removed from the data set once data collection is
completed. Answers will be reported only for the whole group.
