This document is specific to school-level data elements.

The information in this section will be collected for the school once each measurement period.



Data Element Dictionary

The data element dictionary documents definitions, allowable values, sources for information, and instructions for abstraction.

The data elements are designed to provide the necessary data to calculate the measures. A data collection tool will be used for data entry. That tool will incorporate skip-logic. For example, data elements pertinent to specific clinical measures will open only for those schools that offer a relevant telehealth service.

Throughout this document there are references to “the measurement period.” There will be two measurement periods, each six months in duration. These will be January 1 – June 30 and July 1 – December 31.

Variable name:

Name of school, city, state

Variable definition:

The name of the school that is participating in the SB-TNGP

Valid (allowable) values:

Any alphanumeric character

Note for abstractions:

  • Check that the name of the school entered is consistent across measurement periods.

  • SB-TNGP stands for School-Based Telehealth Network Grant Program. SB-TNGP is used throughout this document to indicate services available through this grant.

Variable definition:

The official school enrollment number on the last day of the measurement period

Note for abstractions:

  • If the SB-TNGP telehealth services are only available to certain grades or age groups then only count the school enrollment in the relevant grades or age groups.

Variable name:

Students eligible to participate in SB-TNGP telehealth services

Variable definition:

The number of students eligible to participate in the telehealth services offered through the SB-TNGP on the last day of the measurement period.

Valid (allowable) values:

Any numeric character

Note for abstractions:

  • The SB-TNGP includes the services where telehealth is offered through the grant.

  • Eligible might mean that the student’s parent/guardian has signed a permission form, but we will accept whatever definition the grantee/school uses, as long as it is used consistently throughout the grant period.

  • We are using SB-TNGP as the term to refer to the services offered through the grant because the term “School Health Program” has various meanings to different grantees and we did not want to restrict the definition of eligible students if “School Health Program” conveys some restrictions.

Variable name:

Telehealth services available through SB-TNGP

Variable definition:

The clinical services that are available for delivery via telehealth through the SB-TNGP anytime during the measurement period.

Valid (allowable) values:

Check ALL of the following that apply:

Asthma

Diabetes

Behavioral health

Healthy weight

Oral health

Note for abstractions:

  • Indicate which of the five telehealth services specified in the SB-TNGP Funding Opportunity Announcement (FOA) are available at the school during this measurement period.

  • Check all that apply because multiple telehealth services may be available through the SB-TNGP at the school.

  • This data element captures whether or not a telehealth service is available at the school; it does not require that any particular service is available for specific students or visits, just that the telehealth service is available to someone at the school.

  • Having an ASTHMA service available for students means student has access through SB-TNGP to health care providers who are providing services to monitor and control asthma (e.g., primary care provider, pulmonologist, asthma specialist).

  • Having a DIABETES service available for students means student has access through SB-TNGP to health care providers who are providing services to monitor and control diabetes (e.g., primary care provider, registered nurse, registered dietitian, certified diabetes educator, endocrinologist).

  • Having a BEHAVIORAL HEALTH service available for students means student has access through SB-TNGP to either a licensed mental health professional (e.g., clinical psychologist, clinical social worker, professional counselor, mental health counselor, marriage and family therapist, etc.) or a medical provider trained in providing mental and behavioral health care (e.g., psychiatrist, primary care provider, psychiatric nurse, etc.).

  • Having a HEALTHY WEIGHT service available for students means student has access through SB-TNGP to health care providers who are providing services about nutrition and physical activity to maintain healthy weight (e.g., registered nurse, registered dietitian or nutrition professional, primary care provider).

  • Having an ORAL HEALTH service available for students means student has access through SB-TNGP to oral health providers (e.g., dental hygienist, dental therapist, dentist).

Variable name:

Number of distinct students seen for each available telehealth service

Variable definition:

Indicates the total number of individual students seen for each SB-TNGP telehealth service available during the measurement period (equivalent to unduplicated encounters)

Skip logic:

The response options will be limited to those that match the services available at the school during the measurement period.

Valid (allowable) values:

Enter number of students served for each type of available telehealth service:

Note for abstractions:

  • Students should be counted if they have at least one visit that utilizes telehealth

Indicates the total number of visits delivered using telehealth for each SB-TNGP service available during the measurement period (equivalent to duplicated encounters)
