RE: Privacy Determination for Zika Project

Per our conversation, I’ve determined that the Privacy Act (PA) is applicable to this information collection.  The PA is 
applicable when records are contained within a “system of records”, and those records are retrieved by a personal 
identifier.  In general, to be considered a system of records within the meaning of the PA, records must be retrieved by a 
person’s name or some other “personal identifier”. A personal identifier might include an individual’s name, social 
security number, or any other unique identifier that can be linked to an individual.  
As indicated during our discussion, CDC will be given de‐identified information for each participant; however, each 
participant will also have a unique identifier.  This identifier will be used to identify and retrieve records across various 
project components as the need occurs.    
I’ve also determined that the applicable system of records notice (SORN) for this information collection is: 09‐20‐0136: 
Epidemiologic Studies and Surveillance of Disease Problems. HHS/CDC.
