property within the past 24 months as an owner-occupant. This offer is being submitted with the
representation that I/we will occupy the property as my/our primary residence for at least 12
months.
The undersigned understands that any misrepresentations made on this form as to the agreed to
provisions may be subject to criminal and/or civil penalties including, but not limited to, fine or
imprisonment, or both, under the provisions of Title 18, United States Code, Sections 1001 and 1010.

that HUD will prosecute false claims and statements and that conviction may result in criminal
