Detail/Justification for requested change:
This Non-Substantive Change Request is being submitted to change the status of this collection in ROCIS to a Generic collection. During initial submission of the ICR to HHS/OMB for approval, the radio button on the initial ROCIS screen to make the collection a Generic was not selected. HHS and OMB both reviewed and approved the collection as a Generic ICR. CDC has now gone back and correctly identified the 0920-1235 collection as Generic ICR in ROCIS.
