Revisions:

In an effort to reduce burden on grantees, the Health Resources and Services Administration is removing the following measures from the previously approved information collection request:

  1. Number of individuals who received indirect services during this budget period

  2. Number of positions funded by grant dollars during this budget period

  3. Number of staff who received continuing education or training

  4. Please report the annual program award based on box 12a of your Notice of Award (NOA)

  5. Type of sustainability activities engaged in

  6. Have you developed any of the following: please select all that apply – Sustainability Plan, Business Plan, Communications Plan, Fundraising Plan

  7. Type(s) of technology implemented, expanded or strengthened through this program

  8. Have your organization and/or any of your organization’s providers attested to Meaningful Use?

  16. The percentage of patients 18-75 years of age with diabetes (type 1 and type 2) whose most recent LDL-C test is <100 mg/dL during the measurement year.

  17. Percentage of patients aged 6 months and older seen for a visit between October 1 and March 31 who received an influenza immunization OR who reported previous receipt of an influenza immunization

  18. The percentage of patients 18 - 75 years of age with diabetes (type 1 and type 2) whose most recent HbA1c level is <8.0% during the measurement year.

The following measure are new to this information collection request:

  1. Type of direct service encounters: type of services received by project’s intervention patients as part of project’s quality improvement intervention

  2. Approaches to Quality Improvement: Approaches utilized as part of grant funded quality improvement project implementation

  3. Hospital Utilization Rates: Emergency department utilization (ED) rate and 30-day hospital readmission rate for project’s intervention patient population

  4. Telehealth: Estimated miles saved as a result of using telehealth for project intervention

  5. Agency for Healthcare Research and Quality (AHRQ): AHRQ resources, tools, guidelines, measures, etc. used as part of your grant funded quality improvement project implementation, as applicable.

  6. Statin Therapy for the Prevention and Treatment of Cardiovascular Disease: Percentage of patients - all considered at high risk of cardiovascular events - who were prescribed or were on statin therapy during the measurement period

  7. Comprehensive Diabetes Care: Percentage of patients 18-75 years of age with diabetes who had hemoglobin A1c > 9.0% during the measurement period
