Recently, a survey titled 2018-2019 Survey of Health Care Providers about Family Planning Attitudes and Practices” seeking information on your attitudes and practices related to contraception provision was mailed to you. You were selected randomly from a list of practicing physicians in the United States to participate in the survey.


If you have already completed and returned the survey, thank you. If not, please consider doing so today. The Centers for Disease Control and Prevention (CDC) and the HHS Office of Population Affairs (OPA) will use results to develop educational materials and tools for providers related to family planning service provision.
