In a few days, you will receive a brief survey from the Centers for Disease Control and Prevention (CDC). The CDC is working with drinking water providers that are typical of those found across the country. Your water utility volunteered in order to help us understand all the ways people in your community use water and how water use is linked to health. We are only contacting a small number of residents, and your household has been selected for this important study. You will receive an information packet for the CDC Water and Health Study when the survey arrives.

The survey will take about 12 minutes to complete. You will be able to choose to return the survey by mail or complete it online using a secure website. The survey will ask about your household water use, recent activities, and your health over a 2-week period.

Participation is voluntary, and we hope you will take a few minutes to help us when the survey packet arrives. Your participation can help us learn how to continue to maintain a safe, healthy water supply in your community and across the country. It’s only with the generous help of people like you that our study can be successful!
