All grantees and affiliate delivery sites funded through the DP17-1705 cooperative agreement are required to participate in the CDC-led national evaluation of this effort to scale the National Diabetes Prevention Program (National DPP) in underserved areas and for priority populations. The purpose of this letter is to inform you that the data collection elements for the national evaluation of DP17-1705 have been approved by the Office of Management and Budget (OMB), and the web-based data entry system designed to capture these elements is now available for your use.


You have been asked to participate in this evaluation because of your role in implementing the five required strategies in DP17-1705. We appreciate your willingness to share information and insights about your organization’s efforts to scale and sustain the National DPP for priority populations as we seek to reduce health-related disparities for those populations with the highest risk and disease burden. We plan to use the data from this evaluation to inform our technical assistance and training efforts, including developing and disseminating products through the National DPP Customer Service Center. These products will include emerging/promising practice documents, materials, resources, and tools reflecting data submitted and lessons learned. We will also provide each grantee and affiliate site organization with an individualized feedback report in the form of a performance dashboard.


Through this evaluation, we will be collecting information about:


  • Strategies to recruit sites in underserved areas to deliver the National DPP lifestyle change program

  • Strategies to recruit and enroll participants from priority populations

  • Strategies to hire and train lifestyle coaches

  • Strategies to ensure high rates of participant retention during the yearlong program

  • Strategies to work with healthcare providers/systems to screen, test, and refer priority populations with prediabetes or at high risk for type 2 diabetes to CDC-recognized organizations

  • Strategies to work with employers/insurers to provide coverage for the lifestyle change program

  • Program resources and costs needed to start and implement new CDC-recognized organizations in underserved areas

  • The facilitators and barriers that impact your ability to successfully implement these strategies

 

Completion of the evaluation form is estimated to vary between 5 and 7 hours with an average of 6 hours per affiliate delivery site response, and between 3 and 5 hours with an average of 4 hours per grantee response, including time spent reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information.


Please complete your data submission by December 31th, 2019 through the 1705 National Evaluation Reporting Portal in the Data Reporting for Evaluation and Monitoring of DP17-1705 (1705 DREM) system with your assigned username and password.
