Dear (Evaluator Network Member(s)):
The Centers for Disease Control and Prevention (CDC), Office on Smoking and Health (OSH) is requesting your participation in a web-based survey to assess your level of satisfaction with the Evaluators’ Network. The web-based survey will examine the relevancy and utility of Network services to determine how OSH can better support your surveillance and evaluation needs.
As part of the Web-Based, Evaluators’ Network, Customer Satisfaction Survey, we would like your opinion, as a member of the Evaluators’ Network, to ensure that the diverse perspectives and experiences of states are captured in the services we provide. The web-based will take approximately fifteen (15) minutes to complete. The web-based survey contains multiple-choice answers and, in some areas, and the opportunity to provide additional feedback in a short-answer form about your experience using the Evaluators’ Network. The Evaluators’ Network Member Survey is open from 07/06/2020 – 08/17/2020, a link to survey is below:

Your participation in the web-based survey is entirely voluntary. You may choose to skip questions or stop participating in the web-based survey at any time – discontinuing the survey will not, in any way, impact funding or technical assistance you receive from CDC. The web-based survey results will be recorded to help inform future developments regarding the Evaluators’ Network. All information collected will be kept secure and reported out only in an aggregated form.
The information collected from this assessment will be used to inform decision making aimed at improving the Evaluators’ Network and the services we provide to increase its utility and application among state tobacco control programs. Your participation will ensure that your experiences are considered during the further development of services offered by the Evaluators’ Network.
