Good afternoon Karen,

I have reviewed the documents. It states that no PII is being collected. Apparently, the participants (i.e.,
healthcare providers; adults ages 18-40; adults 25-54; etc.) name and email address will be used only one time
to send the consent forms with no need for follow-up. This information will not be stored on a computer or
Laptop, the site is public-facing and the participants are providing responses to questions regarding opioid
responses, not PII. I concur with column A & B.
