Sheet 1: FPCA

Change Date Justification
Remove national guard check box in section 1 7/19/2018 Causes confusion to national guard members who think they can use this form when they can't because they aren't activated or activated under State orders which is not a UOCAVA population
Took out the examples in section 6. 10/11/2018 Only PR and VT are present, examples don't seem necessary.
Only PR and VT have requirements in sec. 6 10/11/2018 We talked to the states present on the last form and PR and VT are the only ones that require mention here.
Add 2 extra lines to section 6 7/19/2018 More room is helpful for people who need to explain a situation or who have additional requirements
Remove a line from section 3 mailing instructions 7/19/2018 It was not necessary to have the 5th line as addresses have 4 lines and military addresses with five lines usually have name in one of the lines, which is not needed on this form
Add clarification on mail forwarding address in Section 3 7/19/2018 Less confusion for people when writing down their address for mail to be sent to.
Take off AK in section 6 7/19/2018 AK no longer has the requirement
Change wording on principal purpose to say This form serves as an application for registration and/or request for an absentee ballot for elections for Federal office or other elections provided by State law or special provisions for all persons covered by UOCAVA. 10/11/2018 To match FWAB language and to be more clear with the purpose.
Change From Privacy Act to Privacy Advisory 10/12/2018 Privacy and FOIA Officer said that our statement should be an advisory.
Change wording in section one from "my return is uncertain" to "my intent to return is uncertain" 2/7/2019 To clear up the understanding of intent vs. a specific date that a voter must know of their return.
Add the words please see back for instructions after print clearly… 2/7/2019 To make sure that voters look at the instructions.
In Section three header add all caps to the word must. 2/7/2019 To clarify that this section must be filled out.
Section 5 change the words in the header to what are your voting preferences and move select one after how do you want to receive your voting materials… 2/7/2019 To clarify what is being asked of the voter for this section.
In section 5 add A. and B. before the words in the grey boxes to distinguish questions. 2/7/2019 In order to move the voters eye across the section to create better completion rates.
On the instructions page insert after the 3rd number "if you printed the form yourself you can fold the form and seal it in a envelope. 2/7/2019 To make sure voters are sending the printed form correctly.
Add language to the instruction page 1st section 4th bullet, "we recommend you complete this form every year and submit…" 2/8/2019 To clarify the process.
Take off National Guard explanation on the instructions page under number 1. 2/8/2019 To dispell confusion about National Guard being removed from the check boxes in section one on the voter information page.
On instructions page add clarification instructions for section 6 requirements. 2/26/2019 To clarify requirments for section 6 and what listed states need from voters.
