Minor Revisions to the Collection Instrument
SSA is making the following revision:
Change #1:
We are removing the Privacy Act Statements on page 2 of these
forms.
Justification #1: Our Office of the General Council advised us to remove the Privacy Act Statement from these forms as we do not collect personal information, nor do we maintain the information we collect on these forms.
