American Time Use Survey

Because your participation in this survey is so important, attached is your
$40 ATM debit card as a way of showing our gratitude. Please call
1-800-232-1824 on the day listed on the letter enclosed with this card to complete
this one-time survey and to receive the Personal Identification Number (PIN) that
will allow you to activate this $40 debit card. Please have the letter with you when
you call.

How to Use This Card:

1. Once you have completed this one-time survey, the interviewer will give you
a PIN that will allow you to activate the card at most ATMs.
2. Insert your debit card into any ATM displaying the PULSE or CIRRUS logo.
3. The ATM will request your 4-digit Personal Identification Number (PIN).
Enter the 4-digit PIN that your interviewer gave you.
4. When prompted for a transaction, choose “Withdrawal.”
5. The screen may display the following message, “This terminal charges a fee
in addition to any fees your bank may charge. Do you wish to continue?”
Choose “Yes.” (This fee has been prepaid so you will still get the full
$40.)
6. When asked to select the account from which the money should be
deducted, choose “Checking.”
7. Enter $40. (Please withdraw all the cash at once as the card will only work
one time.)
8. Remove the money. You may choose to receive a receipt for your
transaction.
