Notice is hereby given that 46 U.S.C. 10307 makes it obligatory on the part of the master or individual in charge of a vessel when shipping articles are required, to have a legible copy of
the agreement required by 46 U.S.C. 10302, unsigned by the mariner, and without the next of kin information, exhibited in a part of the vessel accessible to the crew.

The seamen agree to conduct themselves in an orderly, faithful, honest, and sober manner, and to be at all times diligent in their respective duties, and to be obedient to the
lawful commands of the master, or of an individual who lawfully succeeds the master, and of their superior officers in everything related to the vessel, and the stores and cargo of the
vessel, whether on board, in boats, or on shore. In consideration of this service by the seamen to be performed, the master agrees to pay the crew, as wages, the amounts beside their
names respectively expressed, and to supply them with the following provisions: at least 3 meals a day that total at least 3,100 calories, including adequate water and adequate protein,
vitamins, and minerals, in accordance with the United States Recommended Daily Allowance.
It is agreed that any embezzlement, or willful or negligent destruction of any part of the vessel's cargo or stores, shall be made good to the owner out of the wages of the person
guilty of the embezzlement or destruction. If an individual holds himself or herself out as qualified for a duty which the individual proves incompetent to perform, the individual's wages
shall be reduced in proportion to the incompetency.
It also is agreed that if a seaman considers himself or herself to be aggrieved by any breach of this agreement or otherwise, the seaman shall present the complaint to the
master or officer in charge of the vessel, in a quiet and orderly manner, who shall take steps that the case requires.

AUTHORITY WHICH AUTHORIZED THE SOLICITATION OF INFORMATION:

PRINCIPLE PURPOSES FOR WHICH INFORMATION IS INTENDED TO BE USED:
A. ESTABLISH A CONTRACT BETWEEN THE SHIPPING COMPANY AND THE CREW MEMBERS PRIOR TO THE BEGINNING OF A VOYAGE AS IS
REQUIRED BY LAW.
B.

PROVIDES THE U.S. COAST GUARD WITH AN OFFICIAL VESSEL ACTIVITY FILE AS WELL AS A COMPLETE LISTING OF THE SEAMEN EMPLOYED
DURING THE VOYAGE, THEIR NAMES, WAGES, ADDRESSES OF NEXT OF KIN, TIME TO BE ONBOARD, BIRTH DATES, CAPACITIES, AND
CREDENTIAL NUMBERS.

C. PART OF THE INFORMATION IS TRANSFERRED TO A FILE MANAGEMENT COMPUTER SYSTEM FOR A PERMANENT RECORD.

THE ROUTINE USES WHICH MAY BE MADE OF THE INFORMATION:
A. TO MAINTAIN RECORDS REQUIRED BY 46 U.S.C. 7319.
B. TO ENABLE ELIGIBLE PARTIES (i.e. the mariner's heirs or properly designated representative) TO OBTAIN INFORMATION.
C. TO PROVIDE INFORMATION TO THE U.S. MARITIME ADMINISTRATION FOR USE IN DEVELOPING MANPOWER STUDIES AND TRAINING BUDGET
NEEDS.
D. TO DEVELOP INFORMATION AT THE REQUEST OF COMMITTEES OF CONGRESS.
E. TO PROJECT BILLET ASSIGNMENTS AT COAST GUARD MARINE INSPECTION/SAFETY UNITS.
F. TO PROVIDE INFORMATION TO LAW ENFORCEMENT AGENCIES FOR CRIMINAL OR CIVIL LAW ENFORCEMENT PURPOSES.
G. TO ASSIST U.S. COAST GUARD INVESTIGATING OFFICERS AND ADMINISTRATIVE LAW JUDGES IN DETERMINING MISCONDUCT, CAUSES OF
CASUALTIES, AND APPROPRIATE SUSPENSION AND REVOCATION ACTIONS.

WHETHER OR NOT DISCLOSURE OF SUCH INFORMATION IS MANDATORY OR VOLUNTARY (Required by law or optional) AND THE EFFECTS ON THE
INDIVIDUAL, IF ANY, OF NOT PROVIDING ALL OR PART OF THE REQUESTED INFORMATION: DISCLOSURE OF THIS INFORMATION IS VOLUNTARY,
BUT SUBJECT SEAMEN MAY NOT BE PERMITTED TO ENGAGE IN EMPLOYMENT ABOARD THE VESSEL FOR WHICH ARTICLES ARE BEING OPENED.
