DEFENSE INDUSTRIAL BASE ASSESSMENT:
U.S. Air Force Supply Chain

SCOPE OF ASSESSMENT
The U.S. Department of Commerce, Bureau of Industry and Security (BIS), Office of Technology Evaluation, in coordination with the U.S. Department of Defense (DOD), U.S. Air Force Sustainment Center (AFSC) is conducting a survey and assessment of organizations affiliated with the sustainment of U.S. Air Force systems supported by the AFSC. The resulting data will provide a baseline understanding of the structure and interdependencies of organizations that participate in Air Force sustainment, maintenance, repair, and overhaul programs and their associated supply chains. This effort will also enhance the AFSC's ability to evaluate and address supply chain deficiencies, foreign sourcing and dependencies, and other challenges facing the U.S. defense industrial base.

GENERAL INSTRUCTIONS
A. Your organization is required to complete this survey on the supply chain for the sustainment, maintenance, repair, and overhaul of aircraft and other U.S. Air Force systems. Your organization has been identified either as a supplier to the Air Force or as part of this multi-tiered supply chain.

You must complete the survey using the DOC/BIS template which is Microsoft Excel based and can be downloaded from the secure Census Bureau portal: https://respond.census.gov/airforcesupplychain

If you are not able to download the survey document, at your request BIS staff will e-mail the Excel survey template directly to you.

For your convenience, a PDF version of the survey and required drop-down content is available on the BIS website to aid internal data collection. DO NOT SUBMIT the PDF version of the survey as your response to BIS. Should this occur, your organization will be required to resubmit the survey in the requested Excel format.
B. Respond to every question. Surveys that are not fully completed will be returned for completion. Use the comment boxes to provide any information to supplement responses provided in the survey form. Make sure to record a complete answer in the cell provided, even if the cell does not appear to expand to fit all of the information. The survey includes a glossary for your convenience, referring to the glossary while completing the survey is recommended.

Sections 3a-3k must be completed before proceeding to respond to later parts of the survey, as answers to these sections support menu options in other survey sections.

DO NOT CUT AND PASTE RESPONSES WITHIN THIS SURVEY. Survey inputs should be completed by typing in responses or by using a drop-down menu. The use of cut and paste can corrupt the survey template. If your survey response is corrupted as a result of cut and paste responses, a new survey will be sent to your organization for immediate completion.
C. Do not disclose any classified information in this survey form.
D. Submission of completed survey documents should be done through the secure Census Bureau portal: https://respond.census.gov/airforcesupplychain
Do not E-Mail surveys to BIS.
E. Questions related to the survey should be directed to BIS survey support staff at [email protected]

E-mail is the preferred method of contact.

You may also speak with a member of the BIS survey support staff by calling (202) 482-7808.

Section 3a: Products and Services
In Section 3 you will be asked to identify the products and services your location manufactures, distributes, researches, and/or provides, or has the capability of providing. For the purpose of this survey "capability" is defined as having the ability to provide the product or service within 12 months in typical business conditions. Products are separated based on Federal Supply Group (FSG) and in each sub-section further differentiated by Federal Supply Class (FSC). For more information on Federal Product Service codes, see https://www.acquisition.gov/PSC_Manual.

Identify each FSG in which your location has capabilities. In the next nine pages (sections 3b through 3k), for each area in which your location has capabilities, you will provide information on the individual type of products or services within these categories in which your location has capabilities.

The list below contains links that can move you to the tab containing that particular FSG. After completing this page you may skip to the sections with the products that pertain to your location, but be sure to review all segments to ensure you do not omit any required information.

Section 4a: Suppliers and Inputs

The first column below will automatically be filled in based on your responses in Section 3 (Federal Supply Classes). For each of your location's products shown below, identify the total number of direct suppliers your location uses in the creation and provision of the product, then provide the information associated with your single most important supplier for that product. For this supplier provide information based on the supplier location providing the product to your location.

For the purpose of this survey, a "sole source" is the only source for the supply of parts, components, materials, or services. No alternative U.S. or non-U.S. based suppliers exist other than the current supplier. Alternates are considered available if they have equivalent technical know-how and production capability, whether qualified or not.

B. Is your location dependent on selling to or funding from USG agencies for its continued viability?

Explain:
What could the USG do to increase your ability to provide high quality products and services at a reasonable cost in a timely manner?

C. Does your location provide any products or services that are export controlled?

Has your location exported any products or services that are export controlled?

Has your location had any deemed exports in the past five years?

Note: A deemed export is the release of controlled technology or information to a foreign person in the U.S. This includes technology made available to foreign nationals for visual inspection, exchanged orally, and made available by practice or application under the guidance of persons with knowledge of the technology.
Would reducing or eliminating some export controls make it easier for your location to provide products/services to the USG?
If yes, pick one product/service for which you would most like to see controls reduced:
Would adding/increasing export controls make it easier for your location to provide products/services to the USG?
If yes, pick one product/service for which you would most like to see controls increased:
Comments on Export Controls:
D. From 2015-2019, has your location received a rated order (DO or DX) from a U.S. Government agency? A rated order means a prime contract, a subcontract, or a purchase order in support of an approved program issued in accordance with the provisions of the Defense Priorities and Allocation System (DPAS) regulations (15 CFR part 700).

Section 7b: Participation in USG Programs
A. Estimate the total number of USG programs your location has directly or indirectly supported in the past four years (including those not listed below):

B. From the list of select U.S. Government programs provided below, identify each program your location has directly or indirectly supported in the past four years, indicating whether your support is for the initial build of the platform (acquisition) or for ongoing support, maintenance, and repairs (sustainment). Then select your location's top five product/services provided for each program supported.

The FSC drop-down list is based on your responses to Section 3. If your product does not appear in the drop-down, ensure that you have indicated in Section 3 that your location provides the product.

If you participate in a USG program that is not listed below, you may enter it manually at the bottom of this page (entries 150-169). In these cases, identify the relevant agency in the 'U.S. Government Agency' column and then identify the program name in the 'Program Name' column.
