U.S. Air Force Supply Chain Survey
The Commerce Department’s Bureau of Industry and Security (BIS), Office of Technology
Evaluation (OTE) is conducting a comprehensive survey and assessment of the U.S. supply chain
network supporting the sustainment of U.S. Air Force programs. This assessment was requested by
the Air Force Sustainment Center (AFSC) at Tinker Air Force Base.

The principal goal of this survey is to gain a better understanding of the domestic supply chain
network which supports the maintenance, repair and overhaul of Air Force aircraft and other
related systems and subsystems. The resulting aggregate data and subsequent analysis will allow
industry representatives and government policy officials to better monitor trends, benchmark industry
performance and raise awareness of potential issues of concern.
Your organization has been identified as a direct or indirect supplier to the U.S. Air Force
Sustainment Center (AFSC), part of the Air Force Material Command. Your location’s participation
is vital to ensure a comprehensive dataset for the assessment. BIS has worked in close partnership with
industry in the past and we anticipate similar cooperation from you on this effort.
To begin completion of your survey, download the Microsoft Excel-formatted survey and related
instructions by clicking on the hyperlinked documents below.

Complete and submit the Excel version via the Census Bureau web portal at
https://bis.doc.gov/AFSurvey. Do not send completed surveys via email. The PDF version is only a
reference document for your internal use and cannot be submitted in place of an Excel survey.
*Note 1: Save a copy of the Excel survey to your computer before you make any inputs.

You are required by federal law (50 U.S.C. § 4555) to complete the survey within 30 days. All
information submitted to BIS is protected as Business Confidential under provisions of the Defense
Production Act (DPA) of 1950, as amended, and will not be published or disclosed, other than in
aggregate form. At no time will such reporting identify your specific organization. Such information
is also protected from disclosure from Freedom of Information Act (FOIA) requests.
