You recently received a notification regarding your participation in the CDC-led national evaluation of cooperative agreement DP17-1705. We informed you that the web-based data entry system was ready to accept your responses to the Office of Management and Budget (OMB) approved data collection for this evaluation. Participation in this evaluation is required as a condition of receiving funding from CDC under DP17-1705.
Our records show that you have not yet completed your data submission. We would greatly appreciate your assistance in providing the requested data as soon as possible. This information will inform CDC’s technical assistance and training efforts by helping to identify best practices for scaling and sustaining the National Diabetes Prevention Program (National DPP) in underserved areas and for priority populations.
Please take the time to complete your submission by accessing the 1705 National Evaluation Reporting Portal in the Data Reporting for Evaluation and Monitoring of DP17-1705 (1705 DREM) system using your assigned username and password. We would like to receive all information by December 31th, 2019.
