Thank you for participating in the first year of the CDC-led national evaluation of DP17-1705, Scaling the National Diabetes Prevention Program (National DPP) in Underserved Areas.
We appreciate the time you took to provide information about your work to implement the required strategies to reach, enroll, and support priority population participants in the National DPP lifestyle change program.
We will use this information to inform our technical assistance and training efforts for all CDC-recognized organizations, including developing and disseminating a variety of tools and resources through the National DPP Customer Service Center.
