INSTRUCTIONS FOR COMPLETING THE LOGBOOK FORM
Complete one line of the daily catch section for each day's fishing. Do not combine multiple trips on a single logbook
page. Complete each column on the form by circling the appropriate selection or entering the appropriate
information. Seven days of fishing can be recorded on each log sheet. INSERT THE FOLDED FLAP OF THE BACK
COVER UNDER THE YELLOW COPY OF THE SHEET BEING WRITTEN ON. This will prevent the entries from
being transferred to the underlying sheets. Sign and date each sheet that is submitted. Record the primary fishing
gear used during the trip.
TOP OF THE PAGE
Enter the Vessel Name, Coast Guard Document Number or State Marine Board Number, Captain's Name, Departure
and Arrival Ports, Port where catch was landed, Departure and Arrival Dates, Total Pounds Landed per trip including all
fish sold and those kept for personal use. A trip begins when the vessel leaves port to begin fishing. A trip ends
when the vessel arrives and sells it's catch. Please do not put multiple trips on a single page. Also record all landing
receipt numbers from all sales.
1. DATE: Enter the date of fishing.
DAILY FISHING SECTIONS
2. FISHING CODE: Circle the appropriate code number identifying the vessel’s major activity during the day.
3. LOCATION (LAT/LONG): Enter the location (to the nearest minute) where most of the fish were caught.
4. TARGET CATCH:
SPECIES NAME: Circle the species name or enter the name of other species that were targeted.
NUMBER KEPT: Enter the total number of the indicated species caught and kept for the day. Enter 0 if no
fish were caught and kept.
DISCARD NUMBERS: Enter the total number of the indicated species discarded (dead or alive) for the day.
5. HRS FISHED: Enter the total number of hours (to the nearest whole hour) that fishing occurred during the day.
6. BYCATCH: Enter the species name, number kept and number discarded of species other than the
target species that were caught during the day.
7. COMMENTS: Record any observations related to fishing activities such as number of gill net vessels seen
during the day, percentage or numbers of catch that had gill net scars, movement of fish, type of feed,
water color, bird flocks, temperature "edges" or "fronts", etc.
8. SIGNATURE: Sign and date each page.

SUBMITTING LOGBOOKS
This logbook is mandatory for all U.S. vessels using hook and line gear to fish for highly migratory species. Vessels are
required to submit original (white forms) within 30 days after each trip to the following address:

The National Marine Fisheries Service (NMFS) is collecting this information in support of its implementation of the
Magnuson-Stevens Fishery Management and Conservation Act, the High- Seas Fishing Compliance Act (HSFCA),
and the Fisheries Management Plan for Highly Migratory Species. The information will be used by NMFS to assess
the status of Pacific highly migratory species stocks and monitor the fisheries. It will be also be used by U.S.
negotiators in international and domestic forums to develop policy for the continued harvesting of Pacific highly
migratory species. Response is mandatory. The information submitted will be treated confidentially in accordance with
NOAA Administrative Orders. The burden for completing a logbook for a fishing trip of 30 days is estimated at 1 hour
above that of logbooks kept for normal business practice. This estimate includes the time for reviewing the
instructions, searching existing data sources, gathering and maintaining the date needed, and completing and
reviewing the collection of information. Notwithstanding any other provision of the law, no person is required to
respond to, nor shall any person be subject to a penalty for failure to comply with, a collection of information
subject to the requirements of the Paperwork Reduction Act, unless that collection of information displays a
currently valid OMB Control Number.
