4. Question 1, page 1, second box, add “(ii)” after “(A)”.
5. Question 1, page 1, second box, add colon after “waiver decision)”.
6. Question 2, page 1, change “entitled” to “titled”.
7. Question 5, page 1, change “you will be expected to file a written brief or statement” to
“and fail to do so the appeal may be summarily dismissed”.
8. Page 1, signature line, add “/Petitioner” to “Appellant”.
9. Form Instructions, ¶ 2. Filing: add “, or U.S. Customs and Border Protection (CBP)” after
“(USCIS)” in first sentence.
