A. Provide a precise written physical description of the article, without reference to the article’s
use, for the proposed duty suspension or reduction to be included in the amendment to
subchapter II of chapter 99 of the HTS.

7. U.S. Customs and Border Protection (CBP) Ruling: Are you aware of any CBP rulings establishing
the HTS subheading classification of your product? (Yes/No) If “yes,” attach a copy of the ruling,
indicating whether it is a published or private letter.
8. Import Documentation: Has your firm previously imported the product? (Yes/No) If “yes,” attach
CBP documentation (e.g., liquidated entry summary, Customs Form 29, or other Customs

documentation) indicating the product’s classification by CBP for each HTS subheading you list
above.
9. Product and Industry Description
A. Provide a short product name for the product. (If the user indicated a renewal “without
change” in response to question 3B, this information will be populated based on the
Miscellaneous Tariff Bill of 2018 (and will not be editable).
B. Provide a brief description of the product and a description of the industry in the United States
that uses the product.
C. Is the product an intermediate good (e.g., is it a chemical intermediate or a component used to
make another product)? (Yes/No)
D. Upload any additional product or industry information. Limit 1 attachment of no more than 5
pages or 25MB. Indicate whether the attachment contains confidential business information.
E. Enter any website address(es) that provide additional information about the product.
