A Completing FSA-894
Applicant should complete an FSA-894 in the administrative county office for all eligible crops affected by hurricanes, floods, snowstorms, tornadoes, typhoons, volcanic activity and/or wildfires.
Follow this table to complete a manual FSA-894.
