This is a reminder that you have been selected by your state education department’s Healthy Schools Program to complete an online survey regarding the Healthy Schools Program Evaluation.
The survey should be completed by the staff person within your agency who is most knowledgeable about your agency’s activities to promote school health infrastructure, training, and technical assistance at district and school levels. If you do not think you are the right person to respond to this survey, please contact Dr. Isabela Lucas (ICF) at [email protected] or 404-592-2155.
The survey should take about 60-75 minutes to complete and will be conducted again in two years. Participants will receive a $20 gift card upon completion of the survey.
