This is a reminder that you have been selected by your principal to complete an online survey regarding the Healthy Schools Program Evaluation.
The survey should be completed by the staff person within your school who is most knowledgeable about the Healthy Schools Program. The survey should take about 60-75 minutes to complete and will be conducted again in two years. Participants will receive a $20 gift card upon completion of the survey.
