On all three forms (NATF 84, NATF 85, and NATF 86), these changes have been made:

On page 1 under INSTRUCTIONS FOR COMPLETING THIS FORM, this sentence was added:

Please enter the Card Validation Code, found on your card, in the space provided on the form (three or four digits).

These sentences were deleted:

Please enter the Card Validation Code in the space provided on the form. For Master Card, Visa, and Discover, this is a three digit code found on the back of the card. For American Express this is a four digit code printed on the front of the card.

In the same paragraph on INSTRUCTIONS…, this sentence was added:

If you provide your email address, we will send you an email with your order number.

This sentence was deleted:

You will receive a postcard acknowledging receipt of your order and providing our tracking number.

On the final pages, the dates were updated from 2015 to 2019 in the Reference Number blocks and Credit Card was changed to lower case credit card.
