Introduction

This collection of information is required to obtain federal funds that will provide transportation options to older adults and people with
disabilities underTitle 49 U.S.C. Section 5310, when the transportation services provided are unavailable, insufficient, or inappropriate in rural
and small urban areas. The funding also provides capital, planning, and operating assistance to states to support public transportation in
rural areas with populations of less than 50,000, where many residents often rely on public transit to reach their destinations under Title 49
U.S.C. Section 5311. The annual total burden is estimated at 54,727 (including the time for reviewing instructions searching existing data
sources, gathering and maintaining the data needed, and reviewing and submitting the collection of information). This information collection
is scheduled to expire on November 30, 2019. Please note that an agency may not conduct or sponsor, and a person is not required to
respond to, a collection of information unless it displays a currently valid 0MB control number. The 0MB control number for this collection is
