________________, is from National Institute for Occupational Safety and Health which is a government research agency that works to improve health and safety and eliminate injuries and illnesses in the workplace.
NIOSH is looking for participants for a study looking at health and safety issues related to haul truck operations and maintenance. The study is taking a systematic approach better understand why haul truck-related accidents continue to be one of the leading causes of accidents and injuries and NIOSH is interested in getting operator perspectives to better understand why safety issues continue to occur. To gather this information, NIOSH would like you to take part in an interview located at or near the mine site. This study will take approximately one hour. If you are interested in participating, I will pass your name and contact information on to the study leader. She will provide you with additional information about the study. Your answers will be kept confidential. If you agree to participate in the study, your name will not be attached to any of your data.
