We are writing to request your participation in a short survey about the national public health accreditation program, implemented by the Public Health Accreditation Board (PHAB). The survey is for accredited health departments, one year following your accreditation decision, and includes questions about accreditation benefits and short-term outcomes. NORC at the University of Chicago is conducting this survey on behalf of the Centers for Disease Control and Prevention (CDC), to assess the benefits and outcomes of the national public health accreditation program.

The survey is estimated to take 20 minutes or less to complete. Your open and honest feedback is appreciated. Findings from this survey will be included in a report to CDC that may be publicly available. In this report, data or quotations will not be linked to the identity of a particular respondent or organization.
