In December 2019, NARA submitted a Change Worksheet to modify the form as follows:
In Section 4, "Classified contract" and "Classified contract #" have “NARA” added to clarify the "contract".
Also, "*Date" has added "visit authorization" to clarify which date.
The check box and phrase "If there is no date clearance expires check here" have been removed.
In Section 10, after "Term" the word "candidate" replaces "employee".
