Attachment 1d. Second Reminder Email for Landscape Assessment Survey
Subject line: 2nd Reminder: Please complete the CDC’s National Diabetes Prevention Program Introductory Session Landscape Assessment
Dear [Name of Point-of-Contact],
A couple of weeks ago, we sent an email asking for your participation in a survey (the Introductory Session Landscape Assessment) of all CDC-recognized organizations and their affiliate class locations that offer the National Diabetes Prevention Program lifestyle change program. We hope you’ll consider participating because your feedback will increase knowledge of recruitment strategies used by CDC-recognized organizations to increase enrollment in lifestyle change programs and will improve understanding of how CDC-recognized organizations are using introductory sessions (also known as “session zeros”).
