The Department of Veterans Affairs received Notice Of Office Of Management And Budget Action (NOA) stating “The agency is required to display the OMB Control Number and inform respondents of its legal significance in accordance with 5 CFR 1320.5(b).”
Specifically, VA must submit a non-substantive change to the survey instrument satisfying the requirement of 5 CFR 1320.8(b)(3)(vi) that VA provide reasonable notice to the respondent on the form or instructions of "The fact that an agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a currently valid OMB control number" prior to fielding the survey.
VA has provided an updated survey instrument containing the required verbiage to address the NOA.
