Privacy Act Statement

1. Authority: Section 202(e) of the Food for Peace Act (7 U.S.C. §1722(e))

2. Purpose: The vegetable oil packaging survey is comprised of questions pertaining to the quality and condition of Food for Peace (FFP) Title II vegetable oil packaging. The purpose of the survey is to collect data on vegetable oil packaging so Food for Peace (FFP) can learn what issues need to be prioritized and addressed.

3. Routine Uses: FFP requests the respondent’s email so survey results may be shared with those who provided feedback. FFP requests affiliation type to ensure all perspectives of the supply chain are represented. FFP is collecting email addresses from each respondent so FFP can share the results of the survey with the respondents after the survey has closed. FFP is collecting affiliation type to track who is completing the survey and what other groups need to be contacted to complete the survey.

4. Disclosure: Email addresses and affiliation are required questions in this survey. Email addresses will not be disclosed or shared; however, affiliation respondent numbers without associated names or email addresses may be disclosed. The survey is not mandatory and individuals can choose to fill it out if they wish to provide feedback to FFP regarding vegetable oil packaging.
