FEMA COVID-19 Donation Web Portal Privacy Act Statement
Pursuant to 5 U.S.C. § 552a(e)(3), this Privacy Act Statement serves to inform you of why DHS
is requesting the information on this form.
AUTHORITY:
The Federal Emergency Management Agency (FEMA) is authorized to collect the information
requested on this form pursuant to 42 U.S.C. 68, 44 U.S.C. 3506, and E.O. 13571.
PURPOSE:
FEMA is requesting this information in order to administer the donation and coordination of
goods in response to the COVID-19 relief efforts. FEMA will use this information to coordinate
the movement of those goods from donors to organizations in need.
ROUTINE USES:
The information requested may be shared externally to organizations in need of these donations
of goods as part of the COVID-19 response. For more information about how DHS handles this
type of collection please see DHS/ALL/PIA-006 DHS General Contact Lists, available at
http://www.dhs.gov/privacy.
CONSEQUENCES OF FAILURE TO PROVIDE INFORMATION:
Providing this information to is voluntary; however, not providing this information may limit
FEMA’s ability to facilitate the donation.
