directs the Office of Housing Counseling
to form a Housing Counseling Federal
Advisory Committee (HCFAC) with
members representing the mortgage and
real estate industries, housing
consumers and housing counseling
agencies. The Membership Application
(HUD–90005) will collect information
for individuals in those groups who
want to serve on the HCFAC. The
information will be used by HUD’s
Office of Housing Counseling to select
and recommend to the Secretary for
appointment the members of the
Housing Counseling Federal Advisory
Committee to ensure the members meet
the requirements of the Expand and
Preserve Homeownership through
Counseling Act and of the Federal
Advisory Committee Act.

mortgage goes into default, the lender
may elect to file a claim for insurance
benefits with the Department. A
requirement of the claims process is the
submission of an application for
insurance benefits. Form HUD 2747,
Mortgagee’s Application for Insurance
Benefits (Multifamily Mortgage),
satisfies this requirement.
