Explanation of Non-Substantive Changes to OMB Control Number: 3060-0355:

• Instructions to FCC Form 492

• Instructions to FCC Form 492A

Purpose of this Submission: This submission is being made for proposed non-substantive changes to an existing information collection pursuant to 44 U.S.C. § 3507. This submission seeks to update (i) the accompanying instructions to FCC Form 492 and (ii) the accompanying instructions to FCC Form 492A. This is due to minor changes in the contact information for the FCC’s Office of the Secretary and Wireline Competition Bureau. On December 11, 2018, the FCC opened the Office of Economics and Analytics, Industry Analysis Division which is now responsible for these data collections.

Summary of Proposed Changes to the FCC Form 492 Instructions and FCC Form 492A Instructions:

• Within Section A. of both instructions, the Office of the Secretary’s room number was updated from “TW-B204” to “TW-A325”.

• Within Section A. of both instructions, the “Industry Analysis and Technology Division, Wireline Competition Bureau, 6-A162” was updated to “Office of Economics and Analytics, Industry Analysis Division, Attn.: Mr. Lorenzo Miller, 445 12^th Street, S.W., Room 6-A226, Washington, DC 20554.”
