Attachment B4: Consent to Participate in Teleconference Interviews

Dr. [Last Name pre-populated], representative of [Institution pre-populated] with ADVANCE Award ID [Award # pre-populated] for [Funding track pre-populated].

PURPOSE OF THE EVALUATION: This evaluation examines the sustainability of ADVANCE strategies after funding ended, as well as the diffusion of these strategies. The findings will inform management and enable programmatic changes. In addition, the findings will inform the development of other NSF programs focusing on sustainability and diffusion.

PURPOSE OF THE TELECONFERENCE INTERVIEW: The information collected in the interview will inform the development of case studies. The interview focuses on the factors that enhanced or impeded the continuation of faculty equity ADVANCE strategies after NSF funding ended. In addition, the teleconference interview will gather information about the ADVANCE strategies that have been diffused within and outside the institution/organization as well as the different venues used to diffuse them.

PROCEDURES: The interview should take approximately 60 minutes. It will be helpful to have notes, evaluation reports, annual reports, and other program materials related to [Institution pre-populated]’s ADVANCE Award [Award # pre-populated] on hand before the teleconference interview.

BENEFITS: Your participation will help NSF leadership and ADVANCE program staff increase their understanding of changes regarding faculty equity ADVANCE strategies at the awardee institution/organization and efforts to disseminate these strategies within and outside the institution. In addition, teleconference interview responses will benefit the ADVANCE awardee community and future applicants by providing information on lasting strategies that address systemic inequities in the academic profession and workplaces.

VOLUNTARY PARTICIPATION: You are under no obligation to participate. Choosing not to participate will have no impact on current NSF awards you may have or on your ability to obtain awards in the future. However, we appreciate your effort in setting aside time to take part in the teleconference interview.

RISKS: We do not anticipate any risks of participating in the teleconference interview. Your decision to participate will not affect current or future relations with the NSF.

AUDIO RECORDING: With your permission, the interview will be recorded to assist in the accurate reporting of responses. Audio recordings will be destroyed after analysis has been completed.

CONFIDENTIALITY: The interview data will be kept and protected on a secure server. Only designated NSF ADVANCE staff and Windrose Vision staff will have access to the data for analysis purposes. Your personally identifiable information (PII) will be removed from the teleconference interview data prior to analysis and will not be publicly shared. The originating grantee institution/organization may be identifiable since only six case studies will be conducted.

CONTACT INFORMATION: For additional information about the NSF study, please contact Dr. Jessie DeAro ([email protected]) at NSF, or Dr. Madeleine Wallace from Windrose Vision (the NSF evaluation contractor) at [email protected] (Telephone number: 703-766-4674).
