INTRODUCTION
We would like to ask you some questions about the hogs and pigs on the land you operate. To understand important issues
in the swine industry, we need to obtain information about the health status of your hogs and swine health problems your
herd may have had, as well as about productivity and management.
This short form will collect information about the total number of hogs and pigs you own and the number of distinct sites that
hogs are raised on. Using information collected on this form, the sites you select will be sent a 2020 NAHMS Swine Large
Enterprise Survey, which will collect important hog management, productivity, and health information. Information needs to
be gathered at the site level because that will give the best quality information regarding the health and management of hogs
in the U.S.
Information collected about your operation are kept confidential, are used only for statistical purposes, and will only be
released in an aggregated form. Response is voluntary, but your response is needed to make regional and national
estimates as accurate as possible. You may find it easier to answer some of the questions if you have your records available.

SECTION 4 – CONCLUSION
1. For each of the selected sites, start a Large Enterprise Survey (LES) by:
a. Writing in the EPAID in the correct box on the title page and
b. Fill in the Site Number with the Site Number from each of the selected sites in Tables 2 and 4 in Section 3 (one per
LES) so that each LES has a unique Site Number.
2. Request a signature on a 2021 NAHMS Swine Large Enterprise-Consent Form for the operation completing this
questionnaire and write in the EPAID in the correct box on the title page.
3. For each of the selected sites start a 2021 NAHMS Swine Large Enterprise-Consent Form by:
a. Writing in the EPAID in the correct box on the title page and
b. Fill in the Site Number with the Site Number from each of the selected sites in Tables 2 and 4 in Section 3 (one
per consent form) so that each 2021 NAHMS Swine Large Enterprise Consent Form has a unique Site Number.
