If appointed, I agree to serve on the (insert name of Board), and to disclose to the Secretary of Agriculture any relationship with any (insert commodity) promotion entity or any organization that has a contractual relationship with the Board.

According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a valid OMB control number.
