NOMINATION FOR APPOINTMENT TO THE PAPER AND PACKAGING BOARD

(Under the Commodity Promotion, Research, and Information Act of 1996, Pub. L. 104-127,110 Stat. 1032 (7 U.S.C. §§7411-7425)

and the Paper and Paper-Based Packaging Promotion, Research, and Information Order, 7 C.F.R. 1222.1-1222.108)

The following statements are made in accordance with the Privacy Act of 1974 (U.S.C. §552a) and the Paperwork Reduction Act of 1995. The authority for requesting the information to be supplied on this form is the Commodity Promotion, Research, and Information Act of 1996, Pub. L. 104-127, 110 Stat. 1032

(7 U.S.C. §§7411-7425). Furnishing the requested information is necessary for the administration of this program. Submission of the Tax Identification Number

is mandatory and will be used to determine affiliation or entity identification.

The Paper and Paper-Based Packaging Promotion, Research, and Information Order (Order) provides that manufacturers or importers of paper and paper-based packaging may nominate manufacturers or importers to serve on the Paper and Packaging Board. Nominees must meet the eligibility requirements as described in §1222.40 and §1222.41 of the Order. Nominated domestic manufacturers must manufacture paper and paper-based packaging in the region (South or Other parts of the U.S.) for which they are nominated. Nominees may be nominated as a manufacturer or an importer, but not both. Domestic manufacturers may only be nominated in one region.
