Hi Danny!
Thanks so much for talking with me this morning! As requested here is the information for our OMB request. The 60
day was just published in the federal register a week or so ago. As we discussed, we are planning to use SurveyMonkey
for two of the surveys (one with recipients and one with subrecipients) and although we will be collecting what
organization they are with, we will not be collecting any information about the individual respondent themselves – just
about the organization and program in general.
So glad that it sounds like it may be an easy process to get approval to use SurveyMonkey! Please let me know if you
have any questions or need any information from me!
