FEEDBACK ON MIXED-DELIVERY SYSTEM AND SERVICE COORDINATION TO INFORM APPROPRIATE, TAILORED, AND TIMELY TECHNICAL ASSISTANCE

OMB #0970-0401

Expiration Date: 05/31/2021

Dear Preschool Development Birth to Five Grantees,

We know that you are currently working on your needs assessments. We also know that service delivery is not an element in this first year of your PDG B-5 grant. Appreciating where you are in your work, we are interested to understand the service delivery structures and approaches that are either in place or that you anticipate will be in place in the near future to inform the provision of technical assistance. Given this interest, we ask you the following questions:

  1. What organizational structures are in place, or are being put in place, to facilitate service delivery coordination at the local level for children and their families (e.g. data sharing agreements, MOUs)?

  2. At this time, will your approach to service delivery coordination be statewide or will it be targeted to specific communities? Please describe how you plan for the PDG B-5 grant funds to support service delivery coordination and how it may reach communities.

  3. Who will you be partnering with to provide a mixed-delivery system, both at the state/territory and local levels, and what services will be included in the mixed-delivery system to support children and their families (e.g. agencies, service providers, government departments)? Please indicate which service will be provided/represented by each partner.

We anticipate it will take around 5 minutes to respond to the questions. Your participation is voluntary and your responses will be kept private.

Thank you so much for sharing!
