This document replaces the staff and visitor COVID-19 screening document previously issued by
ORR on March 13th, 2020.
Until further notice, all staff and visitors must be verbally screened for symptoms of COVID-19
and undergo a temperature check prior to entering any ORR care provider facility. If an
individual answers “Yes” to any of the questions below or has a temperature of 100°F or
greater, their access to the facility must be restricted. Staff and visitors who develop symptoms
while at the facility should leave the facility immediately. Refer to the “Actions” section for
further guidance.
Questions:
1. Today, or in the past 24 hours, have you had any of the following symptoms?1
[YES/NO]
o Fever, felt feverish, or had chills
o Cough
o Shortness of breath or difficulty breathing
o Muscle aches
o Fatigue
o Headache
o Sore throat
o Congestion or runny nose
o New loss of taste or smell
o Gastrointestinal symptoms (i.e., nausea, vomiting, or diarrhea)
2. In the past 14 days, have you had contact with a person known or suspected to be infected
with the novel coronavirus (COVID-19)? [YES/NO]
3. Are you currently under isolation requirements following a positive COVID-19 viral test
result? A viral test checks samples from your respiratory system (such as swabs of the inside
of the nose) and tells you if you have a current infection. [YES/NO]
4. Have you had a viral test for COVID-19, and is the result unknown/pending at this time?
[YES/NO]
5. Does the staff or visitor currently have a temperature of 100°F (37.8°C) or greater?
[YES/NO]
Temperature check procedure: Use no-touch thermometers for staff and visitor temperature
checks. ORR authorizes the use of grant funds to purchase no-touch thermometers. Care
providers may use the thermometers they currently have on hand, following appropriate

thermometer disinfection and infection control procedures between each use, until no-touch
thermometers are procured.
The following is a protocol to safely check an individual’s temperature:
o Perform hand hygiene: Wash your hands with soap and water for at least 20 seconds. If
soap and water are not readily available, use a hand sanitizer that contains at least 60%
alcohol.
o Put on PPE: gown/coveralls, face mask, eye protection (goggles or disposable face shield
that fully covers the front and sides of your face), and a single pair of disposable gloves.
o Check individual’s temperature.
o If performing a temperature check on multiple individuals:
 Use a clean pair of gloves for each individual and thoroughly clean the
thermometer between each check.
 If you are using disposable or non-contact thermometers and did not have
physical contact with an individual, you do not need to change gloves before the
next check.
 If you are using non-contact thermometers, clean them routinely, as
recommended by CDC (https://www.cdc.gov/coronavirus/2019ncov/hcp/infection-control.html).
o Safely remove and safely discard PPE.
o Perform hand hygiene again.
Actions:
Staff






If the staff person reports symptoms (Question 1) or the temperature is 100°F or greater
(Question 5), their access to the facility must be restricted. A staff person who develops
symptoms while at the facility should leave the facility immediately. The staff person
should be advised to go home and follow CDC recommended steps for persons who are
ill with COVID-19 symptoms at this link: https://www.cdc.gov/coronavirus/2019-ncov/ifyou-are-sick/steps-when-sick.html.
If the staff person reports a COVID-19 exposure within the past 14 days (Question 2),
their access to the facility must be restricted. The staff person should be advised to
contact their healthcare provider and/or the local public health authority for advice and
stay home to monitor themselves for symptoms.
If the staff person reports being under isolation requirements following a positive
COVID-19 viral test result (Question 3), access to the facility must be restricted until the
individual has satisfied CDC’s Discontinuation of Isolation for Persons with COVID-19 Not
in Healthcare Settings (at this link: https://www.cdc.gov/coronavirus/2019ncov/hcp/disposition-in-home-patients.html).

If the staff person reports an unknown/pending COVID-19 test result (Question 4),
access to the facility must be restricted until COVID-19 has been ruled out or the
individual has satisfied COVID-19 isolation requirements.

Visitor
 If the visitor reports symptoms (Question 1) or the temperature is 100°F or greater
(Question 5), their access to the facility must be restricted. Visitors who develop
symptoms while at the facility should leave the facility immediately. Facility staff should
recommend that the visitor contact a healthcare provider.
 If the visitor reports a COVID-19 exposure within the past 14 days (Question 2), their
access to the facility must be restricted. Facility staff should recommend that the visitor
contact a healthcare provider and/or the local public health authority.
 If the visitor reports being under isolation requirements following a positive COVID-19
viral test result (Question 3), access to the facility must be restricted until the individual
has satisfied CDC’s Discontinuation of Isolation for Persons with COVID-19 Not in
Healthcare Settings (at this link: https://www.cdc.gov/coronavirus/2019ncov/hcp/disposition-in-home-patients.html).
 If the visitor reports an unknown/pending COVID-19 test result (Question 4), access to
the facility must restricted until COVID-19 has been ruled out or the individual has
satisfied COVID-19 isolation requirements.
Exceptions
 Healthcare worker: If the staff person or visitor is a healthcare worker and reports a
recent COVID-19 exposure (Question 2), the individual may enter the facility to provide
healthcare services if all of the following conditions are met:
1. Per CDC guidelines, their risk exposure category is determined to be low.
2. They remain asymptomatic.
3. Additional precautions are implemented as outlined by CDC guidance
(https://www.cdc.gov/coronavirus/2019-ncov/hcp/guidance-risk-assesmenthcp.html).
 Critical infrastructure worker: If the staff person or visitor is a critical infrastructure
worker and reports a recent COVID-19 exposure (Question 2) , the individual may enter
the facility to provide essential services if all of the following conditions are met:
1. They remain asymptomatic.
2. Additional precautions are implemented as outlined by CDC guidance
(https://www.cdc.gov/coronavirus/2019-ncov/community/criticalworkers/implementing-safety-practices.html).
