Enclosed is a link and secure, unique code to complete your
NAHMS Health Management on U.S. Feedlots 2021 Survey. This
important new study is being conducted by the United States
Department of Agriculture’s (USDA) National Animal Health
Monitoring System (NAHMS) in conjunction with USDA’s
National Agricultural Statistics Service (NASS). Please complete
the questionnaire at your earliest convenience.

Online reporting is fast, costeffective, and secure. All you need is
the 17-digit survey code on the
enclosed form to begin.

Your participation in this study is voluntary, though you are one
of a select number of feedlot producers in your State chosen to
participate in this study and so your participation is valuable.

By mail or fax. Complete the
survey questionnaire and mail it
back in the return envelope
provided or fax it to (855) 4153687.

The study consists of two questionnaires. The secure online form
to complete the Phase I questionnaire is linked to the right. If we
do not hear from you by April 5, 2021, we will attempt to contact
you by phone or by an in-person visit. Upon completion of the Phase I questionnaire, you will be given the opportunity to
consent to be contacted for the second phase of the study, which includes the Phase II questionnaire that will be
administered by an animal health professional representative of NAHMS between June and August, 2021.
How will the data be used?
The current study will be a focused study intended to capture a snapshot of cattle health and management practices on
U.S. feedlots in 2021 and allowing for an evaluation of trends over time in these topics. The outcomes from this study will
be used to obtain more funding to study feedlot cattle health issues and help the U.S. feedlot industry to better know and
serve their producers in outreach, trade, and disease management programs.
Your responses to this survey will be kept confidential and will be available in aggregate form only, ensuring that no
individual operation or producer can be identified, as required by federal law. We will publish the survey findings in the
Health Management on U.S. Feedlots, 2021 report, to be released in Spring of 2022 on the NAHMS website:
http://www.aphis.usda.gov/nahms.
