This is a reminder to complete your National Animal Health
Management System (NAHMS) Health Management on U.S.
Feedlots 2021 Survey.

Complete your survey in one of the following
ways:
Online at www.agcounts.usda.gov.
Online reporting is fast, costeffective, and secure. All you need is
the 17-digit survey code on the
enclosed form to begin.

Your participation in this study is voluntary, though you are one
of a select number of feedlot producers in your State chosen to
participate in this study and so your participation is valuable.

By mail or fax. Complete the
survey questionnaire and mail it
The study consists of two questionnaires. The secure online form
back in the return envelope
to complete the Phase I questionnaire is linked to the right. If we

you by phone or by an in-person visit. Upon completion of the
Phase I questionnaire, you will be given the opportunity to
consent to be contacted for the second phase of the study, which
includes the Phase II questionnaire that will be administered by an
animal health professional representative of NAHMS between June and August, 2021.
