We are writing to ask you to take part in an important National Survey of Drowsy Driving Knowledge, Attitudes and Behaviors (Drowsy Driving Survey) that can help make our nation’s roads safer. Your participation in the survey is completely voluntary.

To help us develop strategies to prevent drowsy driving, the National Highway Traffic Safety Administration (NHTSA) is asking you to complete the Drowsy Driving Survey. Enclosed you will find a small token of appreciation ($2). M. Davis and Company, Inc. (MDAC), a nationally recognized research firm, is conducting the survey on behalf of NHTSA.

Please have the person in your household who is 18 years of age or older, has driven a motor vehicle in the past month, and has the [most recent birthday] [next coming birthday] take the survey.
