CDC UNDERGRADUATE PUBLIC HEALTH SCHOLARS PROGRAM (CUPS) SURVEY PARTICIPANT Invitation Letter
Dear CUPS Alumni,
We hope this email finds you well.
The Office of Minority Health and Health Equity at the Centers for Disease Control and Prevention (CDC) is evaluating the CDC Undergraduate Public Health Scholars Program (CUPS).
We’re inviting you to participate in this evaluation by taking an online survey.
The purpose of this evaluation is to find out how the CUPS program has influenced your career choices and to learn about your school and professional progress since you have completed the CUPS Program.
The information you share will be used to guide future development and planning of the CUPS program and will be published in a journal celebrating the 10th anniversary of CUPS and sharing lessons learned from this pipeline fellowship program.
This survey is confidentiall; no individual survey data will be reported. You will be asked to provide information such as your age, gender, and other demographic information of yourself. The survey information you share will be kept confidential, in a secure, locked database. Your individual responses will not be seen when the results of this evaluation are reported. We are only interested in the aggregated results. The reporting of results will contain no personal information about you nor the institution(s) that hosted you.
This survey will take between 15 and 25 minutes to complete and will be submitted electronically directly to our office.

Your participation in this survey is entirely voluntary. You have the right to change your mind, leave the survey without giving any reason, and without being penalized. Your response to all questions are optional. You may also choose to answer some questions and not others.
Your assistance in providing feedback on the CUPS program will allow us to continue to foster and refine the program in our mission to increase the diversity of our future public health workforce and increase health equity across all populations of American people.
