Good morning,
We are working with the CDC Program Performance and Evaluation Office as external evaluators to assess your experiences with the Evaluation Fellowship Program and the impact that the program has had on your skills and career. It is our understanding that representatives of the program contacted you and let you know that we would be requesting your participation in this important assessment. This year marks the 10th anniversary of the Evaluation Fellowship Program, and we believe that your input can provide vital information to support the development and implementation of the program in the future. We are requesting that you complete a brief survey. Your answers will be confidential. If you are willing to participate in the survey, please click on the link at the bottom of this message.
We also hope to conduct brief follow-up interviews with a small number of Program alumni. The purpose of the interviews is to provide some additional context surrounding the survey responses. You will be given an opportunity to volunteer to be interviewed at the end of the survey.
