Agency Form Number affected by Change Worksheet: AD-3117A - Continuation Sheet For Coronavirus Food Assistance Program 2 (CFAP 2) Milk Production Modification

Other Changes: These changes are necessary to revise the form to include dairy operations starting production after September 1, 2020 which will allow enrollment in CFAP 2.

The specific changes to the AD-3117A Continuation Sheet Form:

1. Item 4 revised to include an option for new and succeeding dairy operations starting milk production after September 1, 2020.

2. Item 5 revised to enter the date the dairy operation started to commercially, market milk.

3. Item 6 revised for dairy operations starting milk production on or after April 1, 2020 until August 31, 2020 will enter pounds of actual milk production.

4. Item 7 revised for dairy operations starting milk production on or after September 1, 2020 will enter the last day of 1^st marketing statement

5. Item 8 revised for dairy operations starting milk production on or after September 1, 2020 will enter actual milk production from 1^st marketing statement

No change to the burden hours for the form.
